The Church of Intoxicatia

Welcome to Alcoholics Unanimous, Church of Intoxicatia. Our organization is an adult social and fraternal group that introduces people and helps create life-long bonds of friendship throughout the United States.

NOTICE: You should only fill out this form if you are a current member of the Church of Intoxicatia. The information provided here will be checked with leadership to verify your status as a member of the Church.

DISCLOSURE: Why we need this information. As a member or initiate of the Church of Intoxicatia, it is of paramount importance that we keep accurate records of those who have joined our organization. Because our structure is both national and local, many functions are placed on our local leadership to plan and promote events for you. This local nature relies on accurate membership counts in each area, and your address is required to determine which Diocese of the Church is best suited to provide you with service.

Your contact information such as phone number and e-mail address is important to keep you up-to-date on Church activities. You will never receive an unsolicited phone call from the Church. You may receive occasional e-mails (approximately 2-3 per month) with updates in regards to your membership in the Church.

This application also includes a series of legal disclaimers that are required as part of your participation in Church activities. These acknowledgements assure a safe environment, and legal protection for other Church members who may wish to organize activities that prevent civil lawsuits. By completing this application for full and active Membership, you agree that you will act responsibly, that others will do the same, and that any misfortune that may befall you is your own, and not the responsibility of the Church or its membership.

SECTION 2: POSITION INFORMATION
Our online application currently registers all users at the position of MEMBER. Currently, requests for promotions should be submitted directly to a leader of the church, or e-mailed to membership@churchofikki.com. If you were a member previous to the new Sponsorship requirements, please enter the year you became a member in the Sponsorship box. This allows us to place your "Member Since" date on your Membership.

SECTION 3: CERTIFICATION
All Members must complete the following certification. By checking the boxes below, you agree to the following statements:

I agree to indemnify and hold harmless the Church, its officials and members for any negative consequences that may arise from my membership, and my voluntary actions within the Church.
I agree that the secrets, rites, and catechisms of the Church are to remain secret, and shall not be revealed to non-members.
I understand that my membership in the Church is extremely voluntary and may be terminated by me at any time, or by the appropriate leadership of the church.
I agree to be an active participant, and if accepting a leadership role, agree to perform the functions as prescribed in the Canon Law of such position.
I understand that the church is a 21 and over organization, and agree never to put the reputation of the church into disrepute.
I agree to be bound in my dealings with the Church by the Canon Laws, and the rites and rituals of the Church.

SECTION 4: ACKNOWLEDGEMENT
I acknowledge that I have completed this application/data change form truthfully and accurately and that its information reflects the most current, accurate details about me. I understand that the Church requires this information to keep track of its membership, and may be shared with other leaders as is required to keep the membership active. I acknowledge that the church may contact me by e-mail or phone on occasion when it is necessary.
